Travel Vaccinations
Please note that travel vaccinations need to be booked at least 8 weeks in advance of your travel arrangements. If you have travel plans and need travel advice and/or vaccinations, please collect or download a travel health questionnaire prior to your appointment and bring it with you when you see the nurse. You can find the questionnaire here.

Access to Medical Records

Under the Data Protection Act 1998 everyone has a right to have a copy of their medical records. There are some exceptions:-

  • Access may be refused if the information contained in the records is deemed detrimental to the health of the patient or another person
  • Details about third parties may be removed
  • If applying for another person you must have their written consent or Power of Attorney

Medical records have been computerised at our surgery since 2003.

Notes from treatment received elsewhere, such as hospitals, are retained there but we usually receive a summary report of your treatment which is added to your records.

If you wish to access your records please apply in writing to the Practice Manager. You will receive a response within 21 days.

A charge of £10 is made if you require information that is held on the computer. If you also wish to access manual records the charge can be up to £50 for the provision of paper copies. Individual sheets are charged at 30p page.
